1UP Surfaces Dealer Application

Do you have experience in the equine or ranching industry, and a solid understanding of working with horses? *
Do you have experience with troweling or similar labor? *
Are you physically able to lift 50 lbs and work on your knees for extended periods? *
Do you have an Ag Exemption? *
All dealers must complete initial training and purchase a starter kit. Training fee is $250/day plus an optional fee of $200 for paw strip installation training. Do you agree to complete required training? *
Are you willing to obtain a small business insurance policy or provide a current certificate of insurance? *

All 1UP Surfaces dealers are required to sign a service agreement that reflects expectations for both parties. Please download the agreement, sign and upload the completed document.
